Safety
No Smoking Policy? Smoking is not permitted anywhere inside any MMCS Clinic facility. There is a designated smoking area with seating outside.

Medical Release Forms
Your medical information is highly confidential, and if you choose to have your information transferred to or from MMCS Clinic, we require that you complete a Release of Information form prior to records transfer. Paperwork fees may apply.
